To receive academic accommodations, students must voluntarily disclose their disability and request accommodations through the Counseling and Career Development Center. Choosing not to disclose will be respected.

Definition of a Disability
Under Section 504 of the Rehabilitation Act and the Americans with Disabilities Act (ADA), a disability is defined as a physical or mental impairment that substantially limits one or more major life activities. Examples include:
- ADHD
- Blindness or Visual Impairments
- Deafness or Hearing Loss
- Learning Disabilities
- Physical or Mobility Disabilities
- Psychiatric or Psychological Conditions
- Speech or Language Disorders
- Traumatic Brain Injury
- Other systemic health impairments
Steps to Request Academic Accommodations
- Schedule an intake appointment with the College Counselor to complete required forms or complete the form below.
- Submit documentation that includes:
- A formal diagnosis and date
- Description of evaluation methods and test results
- Functional impact of the disability
- Current treatments or supports
- Prognosis over time
- Professional credentials of the evaluator
- Documentation Review: The Counselor will evaluate your information in collaboration with instructors or program directors (if needed) to determine reasonable accommodations.
- Accommodation Letter: If approved, the Counseling Center will provide accommodation letters to instructors. Students must repeat this process each semester.
- Appeals Process: Disagreements may be addressed through the college’s Student Grievance/Complaint Procedure (see Student Handbook).
